Papilloedema delaying diagnosis of Wernicke's encephalopathy in a comatose patient.

A case of acute Wernicke's encephalopathy due to hyperemesis gravidarum is described. Florid bilateral papilloedema was present, resulting in diagnostic uncertainty and delay in treatment. Attention is drawn to the rare occurrence of papilloedema in Wernicke's encephalopathy, and possible underlying mechanisms for this physical finding are considered.

Teaching NeuroImages: the full-blown neuroimaging of Wernicke encephalopathy.

A 55-year-old man presented in a global confusional state and after 24 hours fell into deep coma. Laboratory findings, including alcohol level, were normal. MRI showed abnormalities typical of Wernicke encephalopathy (figure).1,2 Thiamine was administered immediately, but the patient died 2 weeks later. The diagnosis was confirmed by measurement of blood thiamine level (19.3 ng/ mL; normal rang...

You are what you eat.

Wernicke encephalopathy consists of a triad of ophthalmoplegia, ataxia, and altered mental status. It is caused by thiamine deficiency and although it is commonly seen in alcoholics, patients undergoing gastric bypass surgery for morbid obesity could be a new group of patients to watch for. Florid papilloedema and optic neuropathy are not commonly seen but are still consistent with the diagnosi...

Wernicke's Encephalopathy in a Patient with Nasopharyngeal Carcinoma: Magnetic Resonance Imaging Findings.

We report a case of Wernicke's encephalopathy in a patient with nasopharyngeal carcinoma with a 3-month history of poor oral intake related to nausea and vomiting due to chemotherapy. The patient later developed deep coma while receiving in-patient therapy. Magnetic resonance imaging of the brain revealed typical findings of Wernicke's encephalopathy. The patient was treated with thiamine injec...

Proton MR spectroscopy in Wernicke encephalopathy.

Wernicke encephalopathy is caused by thiamine deficiency. Although the clinical picture has been well established for some time, clinical diagnosis is attained in only 20% of the cases. MR imaging techniques contribute to early diagnosis of Wernicke encephalopathy. We herein report MR imaging and proton MR spectroscopic findings for a patient with clinical and biochemical features consistent wi...
